Before exploring our programs, take a moment to make sure you meet the basic eligibility requirements.
Most providers qualify, if you meet the requirements below, you’re ready to enroll your caregiver.AFH Provider Requirements
- Be a licensed Adult Family Home (AFH) provider
- Have at least one Medicaid resident currently receiving care
- Have completed the AFHTN Orientation within the past year
- Have access to your ProviderOne authorization list for verification
Caregiver Requirements
- Has been identified for sponsorship (current employee or new hire)
- Has worked at least 8 hours in your AFH prior to enrollment
- Is authorized to work in the United States (I-9 compliant)
- Has completed required background check and fingerprinting for Washington State
